1. Using your login and password information, log onto the "members only" area of the SRM website.
2. Once logged in, click on your name in the left-hand "Profile" index.
3. In the row of gray buttons that appear above your personal information, click the "More Info".
4. Click on the "Survey" link.
5. Check only ONE box in EACH CATEGORY that applies to you.
6. Once you are finished, click the "Update" button in the upper right-hand side of the screen.
